When standing before the gates of the Narzulbur stronghold for the first time, Yatul makes it quite clear that non-Orcs are not welcome to enter. But there is a way for the Dragonborn to enter, by becoming Blood-Kin to the Orcs, by retrieving the Forgemaster's Fingers.

Find the Forgemaster's Fingers
The amulet can be in any one of the following dungeons:
- Ansilvund
- Cragslane Cavern
- Cragwallow Slope
- Cronvangr Cave
- Gallows Rock
- Lost Knife Hideout
- Morvunskar
- Stony Creek Cave
- Uttering Hills Camp
Bring the Forgemaster's Fingers to Mauhulakh
After retrieving the amulet Yatul will let you enter the stronghold, the Chief Mauhulakh will collect the Forgemaster's Fingers and is suitably impressed, welcoming the Dragonborn to the stronghold.
Apparently, this should open the gates of the other Orc strongholds in Skyrim as well, namely Dushnikh Yal, Mor Khazgur, and Largashbur.
Items of Note
- Strangely the Forgemaster's Fingers ends up with Yatul, letting the Dragonborn pickpocket it back.
